We are starting to wind down on Eastern Kingdoms and Kalimdor. We will likely head to Outland soon, although not immediately the split second we turn 58. Outland will wait for us when we finish everything reasonable in the old world.
At this point your quest log should include:
There are two directions we can go at this point. We can do the WPL quests or the EPL quests, and then the other. Overall it works out better to start in WPL, but there's no inn there. So set your hearth in Light's Hope Chapel, and then fly to Chillwind.
Scene 1: Alas, Andorhol (level 57)
Travel down the road to Sorrow Hill and visit Marlene Redpath inside the lone house there, and then check Joseph Redpath's monument in the cemetery next to the house.
Ride back down toward Chillwind Point and travel past it to the western entrance of Andorhol. Approach the guard tower at the beginning of town and walk up to it – but not the opening for the door, or you will aggro the elite inside. Instead, edge around the outside and go to the door frame from the outside to use your beacon torch.
Turn around and go due north to the ruined inn. On the second floor is Chromie, who finishes the next leg in the Redpath quests and starts one of her own. Leave the inn and travel east down either road toward where they reunite and very carefully edge your way into the town hall. Avoid the big mobs of, well, mobs that are around the central fountain. The town hall has a bunch of books on the ground, and one of them is the Annals of Darrowshire. The rest are traps. The trick to identifying the proper book is to zoom in and look very carefully at the pages in the book – the fakes have a gray section, a white section, and then a gray section. The real one is all one color with lines throughout it.
Exit the town hall as carefully as you entered it, and head for the north road out of town. There are two towers at the edge of town. Get the left one first, and then the right one, using the same procedure as before – get right up to the frame of the door without showing yourself to the elite inside.
After clearing tower three, continue east rather than returning to the road. You should see numerous silos that are glowing blue. Clear the area around them, and then use your temporal displacer to spawn temporal parasites. Be well-prepared going into each of these fights. You will get two or three grubs that constantly cast slow on you. They aren't otherwise terribly difficult, but they can overwhelm you in numbers, and sometimes when you kill one another will spawn. Most of the fights will be two or three altogether, but you will likely get one battle where you will need to kill five or more. Getting a partner to work through these may not be a bad idea. Any silo that has blue glowies is ready to spawn some parasites for the quest, and the silos are all over the eastern third of town.
Finish off by approaching the eastern entrance of Andorhol and hitting up the final tower, and then ride through Sorrow Hill to Chillwind Point. Do NOT sell your Bone Fragments from here on out, (or Crypt Fiend Parts, Savage Fronds, or Dark Iron Scraps, but you won't see those here.) Regard them as quest items. Don't confuse these with the Skeletal Fragments you need for the new quest from the alchemist.

Scene 2: Counting the Days (level 57)
Head into the west side of Andorhol and return to Chromie and hand in her two quests. You will receive a follow-up to each, one of which stays in Andorhol. Hack through the various buildings in town looking for metal lunch pails. Recover 5 of these lockboxes, and bring them to Chromie.
Hearth to Light's Hope Chapel.

Scene 3: The Main Event (level 57)
Head to the northeast corner of EPL, into Northdale. This area can be extremely annoying, so jump into the water immediately. The water elementals aren't exactly great either, but better than the enemies on the land that have excellent abilities to work in tandem. Instead, use the Aspect of Neptulon to convert the Blighted Horrors into Discordant Surgers for the Poisoned Water quest. Don't let yourself be knocked out of the water or you'll have more trouble than one water elemental. The Symbol of Lost Honor is at the bottom of the center of the pool. Get the symbol and get out of Dodge.
Head for Northpass Tower, and pass it to the west to talk to Aurora Skycaller. Don't get the follow-up; it won't be worth the trouble for us.
And finally, head into Plaguewood to the west. You have two main objectives here: kill ghouls so you can talk to the Darrowshire spirits afterwards, and collect termites at the colonies throughout the area. The colonies tend to be a little more abundant on the southern side of Plaguewood, however the ghouls will be the harder thing to find since everyone running the quest tries to avoid all the non-ghouls, so their supply is rather limited.
Although that will take a while, when you finally finish, head to Blackwood Lake in the southeast. Dive to the bottom and grab Horgus's Skull. Feel free to grab a few more of the water elemental bracers while you're at it.
From there head south to the Infectis Scar and find the Shattered Sword of Marduk. It's in the northeast corner of the gorge, but you will need to fight or ride your way out.
Ride to Tirion Fordring and complete Of Lost Honor, and then follow into Western Plaguelands to Gahrron's Withering. Beat up on a few grizzly bears in the area as you head into the farm. Be VERY careful around the structures in the farm, as the enemies here tend to cluster near those spots, and you do not want to fight multiples here. The ghosts mana burn you heavily, the banshees debilitate you, and the shades go apes**t at low health on you. However, you do have a second task to do here besides handle the cauldron. Next to the barn is Captain Redpath's Shield. Cautiously retrieve it, noting that there are about 8 mobs in proximity of the silos next to the barn.
One final task. Finish your grizzly killing and head into The Writhing Haunt to finish it. Then off to Chillwind and LHC to finish quests.

Scene 4: The Crusade (level 58)
The Scarlet Crusade has a presence in the northern part of Western Plaguelands, where you have a few quests to take care of now. Head for Dalson's Tears, and ride along the dirt path by the farm, riding past the fields and into the Northridge Lumber Camp. In the northeast corner of the area, down a small path into what looks like it ought to be empty space, is Kirsta Deepshadow. (51, 28)
Kirsta has a quest that you will embark on just as soon as you place your termite jar on the floor of the mill and set the termites loose. Kill High Protector Tarsen first before attempting to fight your way into the mill, and then set the termites on a crate there and right-click it to trigger it.
Return to the road and fight along it to get the kill count for Unfinished Business. You shouldn't need to follow the path up into the hills toward Hearthglen. Return to Kirsta when complete.
Upon receiving the second Unfinished Business quest, go due east (or as close to it as you can) to get on the road to Hearthglen. The path forks partway up, and you will find Huntsman Radley up that fork. Slightly further along the road is a guard tower where Cavalier Durgen hangs out. Pull the two guards at the base of the tower, sneak around the back to get the guy who circles the tower, and then carefully pull out one of the three spellbinders inside using line of sight. You might get two, but you don't want all three…or Cavalier Durgen who patrols up and down the tower. Wipe out the other spellbinder(s) remaining, and then wait for Cavalier Durgen. Once he's dead, climb the tower and repeat the process at the top to get to the Holy Coffer that contains the Mark of the Lightbringer.
Return to Kirsta again, and she will ask you to enter Hearthglen. You have two quests for Hearthglen, and it's completely optional if you choose to do this since Hearthglen is full of elites you are probably not going to solo. However inside the town hall on the left side of town is Davril's Libram, and then climb the tower on the right side of town. Neither are very doable without using some brilliant tactics or a squad to help you.

Scene 5: The Remainder (level 58)
Given that most of the other quests in the Plaguelands go into instances or are otherwise elite, we're going to just do clean-up and move on. By all means, run Scholomance, and Stratholme, and kill Araj and Nathanos, and do the Battle of Darrowshire, and everything else that Plaguelands has to offer if you can find the people and the interest to do so.
Here's your hitlist, in no particular order. Do as many of them as you desire.
Scene 6: Kalimdor (level 58)
With all of the Plaguelands quests either completed or abandoned, there's a very limited supply of things left to do in Kalimdor before we move on to Outland. We'll be hitting Winterspring and then making a short trip to Silithus and that will be that for the Old World.
We only have three quests remaining in Winterspring, and one of them is just to turn it in. So complete Are We There Yeti? and take off down the southern road to Find Rinshalla and dispatch some Enraged Wildkin.
You will eventually see Ranshalla on the left (east) side of the road. Turn right instead into Dun Mandarr and find the broken crate in front of the moonkin cave. Check it, then return to the road and look for a broken wagon there. Finally, inspect the crates next to the wagon. The other piece we need is a random moonkin drop, and we're about to kill some anyway.
Now cross the street and say hi to Ranshalla. Do her escort quest. By the time you finish, you should have the Blue Feather Medallion, but hang out if you don't. Turn in at Starfall Village.
Fly to Darnassus and complete the two quests you have there.

Scene 7: It's Dangerous To Go Alone - optional (level 58/59)
There's an Un'goro quest chain we've had lingering for a long time that we should wrap up. If you decide this quest is too low-level at this point, then skip on ahead and just go to Cenarion Hold in Silithus for Scene 8.
Fly to Felwood and talk to Eridan Bluewind in Emerald Sanctuary. Go kill wildlife for her until you finish the quest and return to her. The drop rate isn't great.
Then fly to Tanaris and find the spot along the shore past the pirate cove where you can summon Aquementas using the stuff in the bag she gave you. Kill Aquementas and return to Marshal's Refuge to talk to J.D. and then Linken.
The final quest in the series is named It's Dangerous To Go Alone with good reason – Blazerunner is no pushover, and you really need to use the special item you have for taking away his weird blue glowy shield that constantly pushes you back. Get another person or three, and fight him INSIDE his cave at the top of Fire Plume Ridge. Make sure after you kill Blazerunner that you search the chest in the cave. Killing him and leaving without finishing the quest really stinks.
Return to Linkin afterwards, and then into Silithus.

Scene 8: Silithus - optional (level 58/59)
Remember at the beginning of the act where we said we were going to do everything reasonable that's left before Outland? There's about 8129734 quests in Silithus, and 8129721 of them are unreasonable. Fortunately, almost all of them require level 60, so just pick up every quest you can find in Cenarion Hold.
Actually, in fairness, all Silithus quests are unreasonable. By the time you're of a level to come here, you probably want to move on to Hellfire Peninsula. So I'm going to label all of these as optional, and you can move on when you like. I got through two of these scenes before feeling like ripping my hair out, so just make sure you're level 59 when you go on.
Oh yeah, here's the map:

Leave town northeast and just start killing everything. Spiders, scorpions, weird upright worm things, the odd air elemental. Don't go into Hive'Ashi, and don't go all the way north to the cultist area, but otherwise anywhere northeast (and somewhat to the northwest) is fair game.
Don't come back until you've finished Securing the Supply Lines and Deadly Desert Venom. The drop rate is pretty crappy, so have a good evening of it! Keep the Sandworm Meat until Outland if you're a cook.

Scene 9: The Crystal Vale - optional (level 59)
Head into the northwest corner of the zone, and hunt the elementals. Additionally, look for the relics on the ground. Go back to town when you finish.

Scene 10: Deathwails - optional (level 59)
Go east to Southwind village and kill 8 druids and 8 sentinels. After every single one, you will get a higher level bug spawn. So 16 kills is really 32 kills. Don't rush.
Then go south of there, and kill all the scorpids and spiders in the area as you reach Ortell's Hideout. Talk to him, and then kill scorpids and spiders on the way back. Finish Noggle's Last Hope, and GTFO out of Silithus before your brain kerplodes.

Finale (level 59)
Ugh, it's over. Finally over.
What? 10 more levels? Aw, crap. Just when I thought we could leisurely enjoy instances like Dire Maul and Blackrock Spire, and finish all those old quests we never did, we have 10 more levels to deal with.
Finish anything and everything you ever wanted to do in Kalimdor and Eastern Kingdoms now. You're going to be, quite literally, abandoning your entire quest log when you start on act 17. (Act 17…how many of these are there gonna be?)

- The follow-up quest to Mark of the Lightbringer is an escort quest to the tomb of Uther. It's doable solo – there are only two ambushes. One is two mobs from Gahrron's Withering, and the other is four zombies from Dalson's Tears.
- Finish Of Love and Family with Artist Renfray on the western road along the island of Caer Darrow. The follow-up is in Stratholme, so you'll probably stop here.
- Finish Poisoned Water, if you haven't already, in Eastern Plaguelands with any of the water elementals.
- Collect the remainder of your Skeletal Fragments, as well as gather 30 Bone Fragments for the Argent Dawn quests that become available at LHC when you reach friendly reputation. The other items you can use for these quests are Savage Fronds, Dark Iron Scraps, Core of Elements, and Cryptstalker Parts.
- Head into Zul'Mashar and kill Hameya.
